      9th Annual Community Care Plan Non-Profit Awards

      2-1-1 Broward is pleased to announce the 9th Annual Community Care Plan Non-Profit Awards presented by Signature Grand and hosted by 2-1-1 Broward. Community Care Plan is the first safety-net, hospital-owned Provider Service Network that serves members enrolled in Medicaid, Children’s Medical Services Network and self-insured employee health plans. The annual awards ceremony celebrates the hard work, dedication and achievement of Broward County’s non-profit organizations and its leaders. Evoking the iconic symbols of a glamorous awards show with a red carpet and searchlights, the awards ceremony and luncheon will take place on Friday, February 22, 2019 at 11 a.m. at Signature Grand in Davie.

      Three finalists, from the 10 award categories, will be announced during the VIP Announcement Party held on January 15, 2019 at Dave & Buster’s in Hollywood. The winners and finalists in each category will receive a monetary grant award for each of their non-profit organizations. Those nominated but not selected as a finalist will be eligible to win a $1,000 grant for their organization through the Publix Community Choice Award. Online voting for the Publix Community Choice Award will be open after January 16, 2019 at www.211-broward.org.

      This year’s event co-chairs include Dr. Deberenia Allen-McKinzie, Medical Director of Population Health of Community Care Plan, and Shelley Eichner, Senior Vice President of Calvin, Giordano & Associates.

      Tickets to the 9th Annual Community Care Plan Non-Profit Awards are $100 per person for general admission, $85 for CLUB 2-1-1 members and $75 per person for non-profit staff/government representatives. After February 1, 2019, all tickets are $100 per person. Table Sponsorships begin at $1,500. Partnership opportunities range from $1,500 to $10,000. Partnerships include unique sponsorships such as $5,000 Golden Statue Partner, $2,500 Lights! Camera! Action! Sponsor and others.
